STEP-BY-STEP GUIDE FOR PARTICIPATING IN WMS ATHLETICS
1. PRINT THE PHYSICAL PACKET AND STUDENT/PARENT ACTIVITY HANDBOOK by clicking here or pick one up from the Athletic Office.
- REMINDER: All physicals must be filled out on our forms. This is mandated by the MSHSAA.
2. GET A PHYSICAL EXAM
If a student intends to play sports for the Washington Blue Jays,
he/she must have a valid physical packet on file in the Activities
Office prior to participation.
- REMINDER: Don't forget to bring your physical packet with you to the doctor so he/she can fill out our forms.
3. FINISH FILLING OUT THE PACKET
After your physical has been completed, make sure the rest of the packet is filled out. This includes:
- Student Signature
- Parent/Guardian Signatures
- Reading the Concussion Information
- In Case of an Emergency Information
4. TURN IN THE PHYSICAL PACKET AND HANDBOOK COMMITMENT FORM to the Athletic Office by the first day of team practice.
5. GET A PRACTICE CARD
Once an athlete has a physical packet on file in the Athletic Office,
he/she is issued a practice card. The athlete will then give this card
to his/her coach in order to participate in practice/tryouts. Without
this card, an athlete will not be able to participate.

Physical Packet
All prospective student-athletes must have a current physical packet
on file in the Washington High School Athletic Office prior to
participation.
WHAT DEEMS A PHYSICAL TO BE VALID (NEW)?: A PPE (physical form) that releases a student to participate in
sports/activities will now be considered valid for two years (730 days). We cannot control when every
student’s physician will want to see them again for a general physical, which will be between the
student’s physician and their parent; however, in order to be permitted to play sports for a school team,
the new MSHSAA bylaw allows a physical to be valid for a two year period (730 days). A current physical
that a student may have on file with the school may be valid for next school year under the new bylaw.
Example: A physical that was received on August 2, 2018 is now valid through August 1, 2020.
Example: A physical that was received May 29, 2018 is now valid through May 28, 2020.
Example: A physical that was received February 2, 2018 is now valid through February 1, 2020.
NOTE: In the event that a student has endured a medical condition
resulting in a significant change with
their medical history the school is advised to request a new PPE
(physical form), or at a minimum,
if/when the school knows that a student has experienced a medical
condition or an injury, the school
shall secure a written medical release from a physician for the student
to be released to return to or to
continue participation. It is recommended that the written medical
release from the student’s
attending physician be retained on file with the student’s most recent
physical form. Only the MSHSAA
PPE Form, the “physical examination”, is valid for a two year period
(730 days). The MSHSAA Preparticipation Documentation such as Medical
History, Parent Permission-Insurance Verification,
Student Agreement, Concussion Education Materials-Parent/Student
Signature, Emergency Contact
Information are still annual requirements that the school must secure
from the students/parents
annually.
- All forms must be on file in the WHS Athletic Office by the first day of team tryouts.
As of Aug. 2012, the Missouri State High School Activities
Association (MSHSAA) has mandated that a physical is valid only if
completed on a MSHSAA physical form. MSHSAA physical forms are available
for pickup in the WHS Athletic Office or can be downloaded by clicking here.
Insurance Requirements
All prospective student-athletes must have basic accident (medical)
insurance coverage to participate in athletics. Insurance information is
to be provided on the MSHSAA physical form.
